Effective June 1, 2021, behavioral health fax numbers for authorizations/precertifications will change. 

Visit https://providerpublic.com empireblue.com and select Forms to access the forms with the updated fax numbers. Please note the following changes and update your records accordingly:

Outpatient fax number: 1-844-452-80-72

Inpatient fax number: 1-844-456-2696
