2023 Modification to Horizon Medicare Advantage Plan programs

Horizon will no longer offer Horizon Medicare Advantage HMO/POS or Horizon Medicare Advantage PPO plans which begin on January 1, 2023. Members currently enrolled in Horizon MA plans should have received a letter in early October to advise of this change and inform them how to enroll in a new Medicare plan.
