EmblemHealth: Post-Acute Care Process Update; Do Not Bill Members with Full Medicaid or QMB

For any services starting on or after December 1, 2022, EmblemHealth will manage post-acute care services for members, replacing eviCore. These services include home health care, skilled nursing facilities, acute rehabilitation, and long-term acute care hospital admissions. To help arrange for post-acute care services and support, you should contact EmblemHealth. Notice that medical equipment for EmblemHealth members and managing ongoing care requests for post-acute care services that began before December 1, 2022, will continue to be handled by eviCore.
