Policy guidelines to be applied to Horizon NJ Health patients with varicose veins/venous insufficiency

Beginning November 30, 2022, policy guidelines (Treatment of Varicose Veins/Venous Insufficiency) will be used when providing services to members enrolled in NJ FamilyCare and Horizon NJ TotalCare (HMO D-SNP) plans.

Horizon will need at least three months of unsuccessful conservative therapy under the guidelines before it deems the following services medically necessary.
