Fidellis Care: Medicaid Risk Charts for Review

Fidelis Care is required by the New York State Department of Health to submit complete diagnostic data for Medicaid members for Risk Adjustment. Fidelis Care has partnered with Optum and CiOX Health to obtain the necessary medical records. If you have received a list of Fidelis Care Medicaid members whose records are needed, you should provide these records as soon as possible.
