FidelisCare has recently introduced several new remittance explanation codes for services rendered to Wellcare Dual Access plan members. These numeric codes are used to indicate additional information determined by the insurance carrier upon claim processing. For example, payers use such codes to indicate a claim denial reason or an out-of-pocket liability. Usually, an appendix to such codes is provided on the last page of the EOB.
