UHC Freedom Plan: New Claims Mailing Address

Before July 1, 2022, all claim submissions and inquiries were sent to Tufts Health Freedom Plan. However, beginning this month all claims are directed to UnitedHealthcare. 

Accordingly, all claims submissions and correspondence for services must be submitted in writing to:
