Something New in the Horizon: Infusion Site-of-Service Policy 
EmblemHealth is committed to delivering appropriate care at a minimum cost for its members. Keeping this in mind they started a new coverage on ongoing maintenance infusion services for adults (21+) as of August 1, 2022.  
This is included in their commercial plans for both EmblemHealth and ConnectiCare. Routine maintenance is covered in outpatient settings like Home (POS 12), Office (POS 11) and Ambulatory infusion suites (POS 24, 49 or POS 12 with Modifier SS).  
Patients with special needs to continue care in a hospital outpatient setting are allowed as an exception. More details are available on their website.
