Provider Directory Updates 

In order to keep the provider directory up to date for their members, AgeWell New York heavily relies on the network providers. Network providers are advised to review the listing in the Provider Directory periodically to make sure that the information is flawless.  

AgeWell’s network provider must serve an advance notice (thirty-day advance notice is recommended) of any demographic changes including office address in detail, office phone number, panel status, or language capabilities. This will ensure that AgeWell New York and its members always have up-to-date provider information at any given point in time.  

Providers are allowed to communicate the changes on office letterhead or by completing the provider change form available on the AgeWell New York website.
