Accurate data is critical for compliance with state and federal regulations, which require Horizon to contact providers quarterly to verify their information. In 2025, Horizon will streamline this process, allowing providers to only verify and attest to the accuracy of their demographic information on an annual basis.

While Horizon will continue to reach out to participating healthcare providers quarterly, providers are responsible for keeping their data current throughout the year whenever changes occur.

Tools for Updating Information

To assist providers in meeting the annual attestation requirement, Horizon offers several tools:

Provider Data Maintenance Tool: A resource for managing and updating provider information.

Horizon Data Submission Template for Practitioners: A straightforward template for submitting updates.

CAQH Provider Data Portal: This portal, formerly known as CAQH ProView™, allows providers to review, update, and re-attest to the accuracy of their information. Providers who are not yet registered can visit CAQH.org to get started.
