This fictionalized scenario is based on aggregated reports and interviews with practice administrators and does not represent a single documented case. What the practice manager didn’t know was that their application had been flagged by an AI system for “anomalous billing patterns” based on data from two years ago, automatically routing it to a denial queue where it would remain indefinitely.
