What Every Provider Needs to Know This Month 

If there is one section of this newsletter you read in full, make it this one. This month’s insurance updates are unusually dense with changes that carry real financial consequences — and several of them are already in effect or have deadlines coming up fast. 

The most urgent item: if you have a provider billing diagnostic imaging or radiology to VillageCareMax Medicare plans, nearly every code now requires prior authorization — with no official notice from the payer and no grid available online. The only way to know is to check, and the only way to avoid denials is to act now. 

On the billing side, both Anthem and Fidelis Care have independently addressed modifier 25 practices — Anthem is explicitly excluding CPT 99211 from reimbursement when billed with modifier 25 under Medicare Advantage effective April 1, while Fidelis Care is reminding providers of the 50% reimbursement rule for problem-oriented E&M billed alongside procedures or preventive visits. If your providers bill same-day E&M regularly, this month’s updates are required reading. 

For facilities, EmblemHealth is cutting reimbursement for inpatient transfer discharge days and eliminating payment for same-day transfers entirely starting July 15 — a change with direct revenue impact for any facility that transfers patients to acute care settings. Horizon is retiring H0032 for ABA services on the same date, with a transition window running through January 2027. 

On the compliance front, Horizon’s RADV audit warning is not one to ignore. CMS has resumed multi-year audits reaching back to 2017, and incomplete documentation means diagnosis disallowance and potential repayment. Fidelis Care’s new Quality Policy goes further — providers who fall below minimum HEDIS and Stars thresholds now face contract termination. 

There are also two hard deadlines this month that require immediate action: HETS vendor enrollment must be completed by May 11 or eligibility verification workflows will be disrupted, and the ACCESS Model application window closes May 15.

VillageCareMax: Assume all diagnostic imaging and radiology codes require authorization for Medicare plans — effective now, no official notice issued.

RADV audits: CMS is auditing back to 2017. Documentation must be complete, signed, dated, and include provider credentials — or risk diagnosis disallowance and repayment. 

Fidelis Care Quality Policy: New minimum performance thresholds are now contractual. Providers below 2.5 Medicare Stars or below 75th percentile on 50% of Medicaid HEDIS measures face potential termination. 

NYS Medicaid revalidation: Paper revalidations are gone after May 1. If you were notified between February and April and haven’t submitted yet, hold — PSP instructions are coming. If you submit paper after May 1, eMedNY will return it unprocessed. 

HETS deadline — May 11: Enroll third-party eligibility vendors to your NPI or lose eligibility verification access. 

ACCESS Model — May 15: Last chance to apply before the July 5 launch. Includes a new co-management billing opportunity for clinicians. 

Essential Plan 200-250: Coverage ends June 30. Help affected patients update their NY State of Health accounts and prepare for transition now. 

Aetna RPM: Coverage now limited to heart failure, hypertension, and diabetes only. Providers monitoring other conditions have until August 31 to transition affected patients. 

Healthfirst fee schedule: Rates shifted to locality-based CMS percentages March 1 — several high-volume E&M codes decreased. Review your revenue projections. And if you bill HealthFirst as an office-based provider, audit your 2026 remittances now: a platform migration error has been systematically applying facility rates to non-facility claims since February, and the underpayments won’t show up as denials. 

Benefit Funds / ClaimsXten: New auditing platform is live. Expect more systematic claim edits — coding precision is now more important than ever.
