Healthcare organizations have spent a decade hardening their perimeters. Firewalls. Encryption. Multi-factor authentication. Incident response plans rehearsed and revised. The investment is real, and the threat it addresses is real.
But a different kind of vulnerability has been building in parallel — one that has nothing to do with external attackers and everything to do with what the record says and whether it gets where it needs to go. Federal regulators have spent the past several years constructing an enforcement framework around exactly this issue. Most organizations have not caught up.
