AI in Interventional Radiology: What Can Practices Actually Bill?

Reimbursement pathways, payment limitations, and financial considerations for VIR practices

By Elina Sabilova, Billing Department, WCH

Every interventional radiology group has had some version of the same conversation. A vendor demonstrates an AI tool, the images look impressive, and someone eventually asks the question that matters to the practice: Who pays for it?

The answer depends on what the technology does, where it is used, how the service is coded, and whether a specific payment pathway applies. An AI tool may support image interpretation, quantify anatomy, assist with procedural planning, or generate a clinical recommendation. Those functions do not automatically create a separately billable service.

For vascular and interventional radiology (VIR) practices, the challenge is to distinguish clinical utility from reimbursable work. Understanding that distinction before purchasing or deploying a tool can help practices avoid unsupported claims, unexpected costs, and compliance risk.
