MLTC Plan Enrollment and CBLTSS saw NY State’s Independent Assessor from May 16, 2022
The need for Community Based Long Term Service and Supports (CBLTSS), Consumer Directed Personal Care Services (CDPAS), and enrollment into a Managed Long Term Care plan are based on the completion of a Community Health Assessment (CHA) and Clinical Appointment (CA) by an independent practitioner. The independent practitioner also completes the Physician’s Order through NY Stat’s Independent Assessor.
