Empire: New Policy for Transitional Care Management

Empire BlueCross BlueShield (Empire) always strives to add value to Transitional Care Management. Hence, Empire has launched a new reimbursement policy to encompass discharge from the emergency room to help prevent any unwanted incidents during the transition. 

Back on April 27 this year, Empire implemented a new professional reimbursement policy termed as Transitional Care Management (TCM). The TCM period begins upon the member’s discharge from the emergency room to a community setting with a validity of 29 days.
