Horizon NJ Health and eviCore healthcare are in a partnership. The aim is to help manage the radiation therapy services by conducting pre-service MND reviews as part of the Radiation Therapy Program.
Based on recent FDA approval, for services to be rendered on and after August 1, 2022, eviCore will also conduct a pre-service MND review of Pluvicto™ for the treatment of adult patients with PSMA-positive metastatic castration-resistant prostate cancer (mCRPC) who have been treated with AR pathway inhibition and taxane-based chemotherapy.
You can submit pre-service MND requests for Pluvicto using CPT code A9699 and NDC 6948-010-61.
