Allergen immunotherapy 

Effective January 1, 2023, home-based, self-administered subcutaneous allergen immunotherapy will no longer be covered. This will apply to all participants in the UnitedHealthcare program. The medical policy for immune therapy of environmental allergens will be updated to reflect this change. Effective January 1, 2023, claims for serum mixtures of immune therapy for environmental allergens will only be covered under CPT® codes 95165 or 95199. They are prepared and administered in a clinician’s office under the supervision of a trained healthcare professional, as prescribed in medical policy.
