CMS Is Coming for Cardiologists: What the New Heart Failure Payment Model Means for Your Practice 

A mandatory five-year program starts January 1, 2027 — and 2,600 physicians are already on the list 

CMS has released a preliminary list of 2,600 cardiologists who will be required to participate in the new Ambulatory Specialty Model (ASM) for heart failure starting January 1, 2027. The five-year mandatory program runs through December 31, 2031, and will hold participating physicians financially accountable for the quality and cost of care for their Medicare heart failure patients.
