To support this commitment, Empire requires all contracted providers to participate in annual training on the Model of Care for Empire, designed to enhance understanding and engagement with the SNP program. This comprehensive training covers vital aspects of SNPs, including cost sharing, data sharing, Interdisciplinary Care Team (ICT) participation, health risk assessment results, plan of care, and benefits coordination.
